| 目的 探讨AIDS相关肝脏淋巴瘤的超声特征性表现，以提高对该病的认识。方法 回顾性分析5例经病理证实的AIDS相关肝脏淋巴瘤患者的超声影像及临床资料。结果 5例患者均为弥漫大B细胞淋巴瘤，均合并全身其他器官或淋巴组织淋巴瘤，血清LDH明显升高，肿瘤标志物癌胚抗原、糖类抗原199及AFP值均在正常范围。5例患者中，4例呈多发结节型，1例呈单发肿块型；多发结节型表现为肝内多个低-极低回声结节，部分结节回声接近囊肿样无回声；单发肿块型表现为肝右叶巨大低回声，内见血管穿梭征；病灶后方回声均轻度增强；彩色多普勒血流显像提示病灶内部可见少许条状血流信号；5例患者肝静脉、门静脉、胆道未见瘤栓。结论 AIDS相关肝脏淋巴瘤有特定的超声声像图特征，结合LDH升高及肿瘤标志物阴性情况，可提升肝脏淋巴瘤的诊断准确率。 |

| Objective To investigate the ultrasonographic features of AIDS-related hepatic lymphoma and to improve the understanding of this disease. Methods The ultrasonographic image and clinical data of 5 patients with AIDS-related hepatic lymphoma confirmed by pathology were analyzed retrospectively. Results All the patients were diagnosed as diffuse large B-cell lymphoma, combined with other organs or lymphoid tissues lymphoma. The level of serum LDH was significantly increased and the levels of tumor markers carcinoembryonic antigen, carbohydrate antigen 199 and AFP were normal. Among the 5 patients, 4 cases were multiple nodules and 1 case was single mass. Multiple nodules were manifested as multiple hypoechoic or extremely hypoechoic nodules in liver, some nodules echoed nearly as cystic anechoic nodules. Single mass was characterized by giant hypoechoic mass in the right lobe of liver and vascular shuttle sign was visible. The posterior echoes of the lesions were slightly enhanced. Color doppler flow imaging: a little strip blood flow signal could be seen within the lesion. No tumor thrombus was found in hepatic vein, portal vein and bile duct in 5 patients. Conclusions AIDS-related hepatic lymphoma has specific ultrasonographic features. In combination with the elevated LDH and negative tumor markers, the diagnostic accuracy of hepatic lymphoma can be improved. |
