      目的 探讨茜草配方颗粒治疗耐多药肺结核(multi-drug resistant pulmonary tuberculosis, MDR-PTB)的临床效果及对免疫功能的影响。方法 选取2015年6月—2018年5月宜宾市第二人民医院感染二科收治的136例MDR-PTB患者为研究对象,采用随机数字表法将其分为观察组和对照组,各68例。对照组予以中国全球基金耐多药结核病项目标准治疗方案治疗;观察组在对照组治疗方案基础上予以茜草配方颗粒治疗。比较2组临床疗效、痰菌转阴率、免疫功能改善情况及不良反应发生情况。结果 治疗后观察组总有效率高于对照组(P<0.05);治疗3、5、8个月后观察组痰菌转阴率均高于对照组(P均<0.05);治疗后,观察组血清IgA、IgG、IgM水平均高于对照组(P均<0.05);观察组CD4+ T细胞、CD4+ T细胞/CD8+ T细胞水平均高于对照组,CD8+ T细胞水平低于对照组(P均<0.05);观察组肝功能异常发生率低于对照组(P<0.05)。结论 对MDR-PTB患者予以茜草配方颗粒联合标准抗结核化学药物治疗方案治疗可有效提高临床疗效及痰菌转阴率,调节机体免疫功能,且安全可靠。

      Objective To investigate the clinical effect of formula granule of Rubiae Radix et Rhizoma in the treatment of multi-drug resistant pulmonary tuberculosis (MDR-PTB) and its influence on immune function. Methods Totally 136 MDR-PTB patients admitted to the Second Department of Infectious Disease, the Second People’s Hospital of Yibin from June 2015 to May 2018 were selected as the study subjects. The included patients were divided into observation group and control group by using random number table, with 68 cases in each group. The control group was treated with standard treatment regimen of Global Fund Multi-drug Resistant Tuberculosis Program of China. The observation group was treated with formula granule of Rubiae Radix et Rhizoma on the basis of the control group. The clinical effect, sputum negative conversion rate, improvement of immune function and incidence of adverse reactions were compared between 2 groups. Results After treatment, the overall effective rate of the observation group was higher than that of the control group (P<0.05). After 3, 5 and 8 months of treatment, the sputum negative conversion rate of the observation group was higher than that of the control group (P<0.05). After treatment, the levels of serum IgA, IgG and IgM in the observation group were higher than those in the control group (P<0.05). The levels of CD4+ T cells and CD4+/CD8+ T cells in the observation group were higher than those in the control group, and level of CD8+ T cells was lower than that in the control group (P<0.05). The incidence of abnormal liver function in the observation group was lower than that in the control group (P<0.05). Conclusions The combined treatment of formula granule of Rubiae Radix et Rhizoma with standard anti-tuberculosis chemotherapy regimen in MDR-PTB patients can effectively improve the clinical effect and sputum negative conversion rate and regulate immune function. It is safe and reliable.
