      探究微小RNA-1285-3p(microRNA-1285-3p, miR-1285-3p)在肺结核患者外周血单核细胞中的表达及其临床意义。方法 选取2017年7月—2021年3月在我院诊治的146例肺结核患者作为研究对象(肺结核组),其中潜伏结核感染(latent tuberculosis infection, LTBI)患者85例(LTBI组),活动性肺结核(active pulmonary tuberculosis, APTB)患者61例(APTB组),另纳入同期健康体检者146例为健康组。采用实时荧光定量PCR(real-time fluorescence quantitative PCR, qRT-PCR)法检测肺结核组和健康组单核细胞中miR-1285-3p表达水平;采用ELISA法测定血清TNF-α、IL-1β水平;采用Pearson法分析APTB患者单核细胞中miR-1285-3p表达水平与血清TNF-α、IL-1β水平的相关性;采用ROC曲线评估单核细胞中miR-1285-3p表达水平诊断APTB的价值。结果 肺结核组单核细胞中miR-1285-3p表达水平低于健康组,血清TNF-α、IL-1β水平高于健康组(P均<0.05)。APTB组患者单核细胞中miR-1285-3p表达水平低于LTBI组,血清TNF-α、IL-1β水平高于LTBI组(P均<0.05)。APTB组患者单核细胞中miR-1285-3p表达水平与TNF-α、IL-1β均呈负相关(r分别为-0.588、-0.569,P均<0.05)。单核细胞中miR-1285-3p水平诊断APTB的AUC为0.859(95%CI:0.795~0.923),截断值为0.56,此时敏感度为81.2%,特异度为88.5%。结论 肺结核患者外周血单核细胞miR-1285-3p表达水平下调,APTB患者miR-1285-3p表达水平下调更甚,且其与TNF-α、IL-1β等炎症因子相关,测定单核细胞中miR-1285-3p表达水平有利于临床诊治APTB、评估肺结核疾病进展程度。

      To explore the expression of microRNA-1285-3p (miR-1285-3p) in peripheral blood monocytes of patients with pulmonary tuberculosis and its clinical significance. Methods A total of 146 pulmonary tuberculosis patients diagnosed and treated in our hospital from July 2017 to March 2021 were selected as the pulmonary tuberculosis group, including 85 patients with latent tuberculosis infection (LTBI) (LTBI group) and 61 patients with active pulmonary tuberculosis (APTB) (APTB group), and 146 patients with normal physical examination results during the same period were included in the healthy group. The expression level of miR-1285-3p in monocytes of pulmonary tuberculosis group and healthy group was detected by real-time fluorescence quantitative PCR (qRT-PCR). ELISA method was used to measure the levels of serum TNF-α and IL-1β. Pearson method was used to analyze the correlation between the expression level of miR-1285-3p in monocytes of APTB patients and the level of serum TNF-α and IL-1β. ROC curve was used to assess the value of miR-1285-3p expression level in monocytes for the diagnosis of APTB. Results The expression level of miR-1285-3p in monocytes in the pulmonary tuberculosis group was lower than that in the healthy group, and the levels of TNF-α and IL-1β in serum were higher than those in the healthy group (P<0.05). The expression level of miR-1285-3p in monocytes of patients in the APTB group was lower than that in the LTBI group, and the levels of TNF-α and IL-1β in serum were higher than those in the LTBI group (P<0.05). The expression level of miR-1285-3p in monocytes of patients in the APTB group was negatively correlated with TNF-α and IL-1β (r=-0.588 and -0.569, respectively, P<0.05). The AUC of miR-1285-3p level in monocytes for diagnosing APTB was 0.859 (95%CI: 0.795-0.923), and the cut-off value was 0.56. At this time, the sensitivity was 81.2% and the specificity was 88.5%. Conclusions The expression level of miR-1285-3p in peripheral blood monocytes of pulmonary tuberculosis patients is down-regulated, the expression level of miR-1285-3p in APTB patients is obviously down-regulated, and it is related to TNF-α, IL-1β and other inflammatory factors. Determining the expression level of miR-1285-3p in monocytes is conducive to the clinical diagnosis and treatment of APTB and the evaluation of the progression degree of pulmonary tuberculosis.
