目的:探讨 ADC值与肝细胞癌(HCC)组织学分级的相关性以及不同肿瘤直径中ADC值与HCC的相关性的差别。
方法:回顾性分析2017年-2020年180例病理证实为HCC的病例,按大小分为<2 cm、≥2 cm且<3 cm、≥3 cm且<5 cm、≥5 cm4组,标为I、II、III、IV组。分析ADC值与HCC组织学分级的相关性,并分析在不同肿瘤直径中ADC值与HCC的相关性是否有差别。
结果:高、中和低分化HCC的ADC值分别为1.159±0.302×10-3、0.951±0.213×10-3和0.811±0.239×10-3 mm2/s,单因素方差分析方法显示3组ADC值之间存在差异(F =26.229,p<0.001),且两两比较均有统计学意义。ADC值与总体HCC的组织学分级呈负相关(r = -0.474,P<0.05),与I-III组HCC的组织学分级均呈负相关,分别为(r = -0.663,P< 0.05)、(r = -0.527,P< 0.05)、(r = -0.364,P< 0.05),且均有统计学意义,然而与IV组HCC的组织学分级相关性没有统计学意义(r = -0.292,P> 0.05)。
结论:ADC值可能是非侵入性地预测HCC组织学分级的有用工具,预测价值受肿瘤大小影响,它或许更适用于小HCC,对于>5 cm的HCC提供的价值可能有限。 |

Objective:To investigate the correlation between ADC value and hepatocellular carcinoma grade and the difference between ADC value and HCC in different tumor diameters.
Methods: A retrospective analysis of 180 cases of HCC confirmed pathologically from 2017 to 2020 were divided into four groups according to size :<2 cm、≥2 cm且<3 cm、≥3 cm且<5 cm、≥5 cm , labeled as group I, II, III, and IV. Analyze the correlation between ADC value and HCC histological grade, and analyze the difference between ADC value and HCC is in different tumor diameters.
Results: The ADC values of high, moderate and poorly differentiated HCC were 1.159±0.302×10-3, 0.951±0.213×10-3 and 0.811±0.239×10-3 mm2/s respectively.Single-factor analysis of variance showed that there were significant differences among the three groups (F = 26.229, P< 0.001), and the pair wise comparisons were statistically significant. The ADC value was negatively correlated with the histological grade of HCC in general (r = -0.474, P<0.05), and negatively correlated with the histological grade of HCC in groups I-III,respectively, there was no significant correlation between HCC grade and histological grade in Group IV (r =-0.292, P > 0.05) .
Conclusion: ADC value may be a useful tool for non-invasively predicting HCC histological grade. The predictive value is affected by tumor size. It may be more suitable for small HCC, and its value may be limited for HCC> 5 cm. |
